Step by Step Guide for Paper Lunch Application 

You can obtain a paper application from the cafeteria at the school that your children attend. If you have more than one child that attends Brunswick County Schools, you only have to fill out one application with all your children's name. Please make sure that if you fill out a paper application that is for the current year and not the previous year. 

Steps to fill out Paper Lunch Application

In Section A of the paper application, please fill out the following:

  • Students name
  • Circle S for Current Student or O for Other Sibling
  • School name 
  • Grade 
  • Circle H for Homeless, M for Migrant, R for Runaway, or F for foster if this applies to your child
    • If your child is Foster, then proceed to Section E
  • Student income from work 
  • Student income from other sources

 In Section B of the paper application, please fill out the following:

  • Check mark yes or no to the question if your family participates in SNAP, TANF or FDPIR
  • If the answer is yes, then please provide us with the Case Identifier that you can obtain by calling Brunswick County DSS at 910-253-2077
    • The case identifier is not your EBT number
    • Once you obtain Case Identifier, please write the Case Identifier Number in the box asking for case number.
    • Once you fill out case number you can proceed to Section E
  • If the answer is no, then proceed to section C

 In Section C of the paper application, please fill out the following:

  • Head of Households name
  • Earnings from work 
    • (Circle the Frequency of how it is earned- Weekly, Monthly, Bi-Weekly, and or Bi-Monthly)
  • Earnings earned from Public Assistance/Alimony/ Child Support
    • (Circle the Frequency of how it is earned- Weekly, Monthly, Bi-Weekly, and or Bi-Monthly)
  • Earnings earned from Pensions/Retirement /All other Income
    • (Circle the Frequency of how it is earned- Weekly, Monthly, Bi-Weekly, and or Bi-Monthly)

 In Section D of the paper application, please fill out the following: 

  • Total Household Members listed on the application
  • Last four digits of the head of the household social security number

 In Section E of the paper application, please fill out the following:

  • Signature/Printed name of the Head of Household
  • Date you filled out application
  • Email address
  • Mailing Address
  • Contact number 

 Section E is the last section that you need to fill out.
